Jay P. Slotkin, MD, MPH, CMD, AGSF, FACP
Dr. Slotkin is a new associate with the Center for Value-Based Medicine. His long standing dedication and practice to the health and welfare of adult and geriatric persons combined with his interest and experience in geriatric assessment and long term care needs are strong assets to the Center’s group.
Dr. Slotkin has been a physician in the private practice of adult and geriatric medicine for 25 years. As the founder and chief executive of East End Geriatric and Adult Medicine, Dr. Slotkin collaborates with nurse practitioners and other medical internists in caring for patients in office, hospital, nursing home and home care settings. He is also the co-founder of the Southold Geriatric Network, an organization that links other geriatric professionals in collegial and professional interaction on an ongoing basis.
Dr. Slotkin is a Clinical Associate Professor in the Department of Medicine, at SUNY Medical School at Stony Brook, NY. In that capacity, he serves as a preceptor to Primary Care Internal Medicine Residents and medical students, lectures in Geriatrics as well as precepting Geriatric Physician Assistants in their clinical education.
Dr. Slotkin earned a M.D. from SUNY Upstate Medical Center in Syracuse, NY and an M.P.H. from University of North Carolina at Chapel Hill, NC. Dr. Slotkin serves as the Medical Director for the San Simeon Center for Nursing and Rehabilitation and has published on the topics of geriatrics and geriatric home care. He is Vice President of the New York Medical Director Association and remains active in his professional organizations, including American Geriatric Society and American Gerontological Association.
